To register with the surgery:

• use the NHS Register with a GP: Bluebell Surgery service.

To register a new patient you will need to live within our practice boundary and will need to bring two forms of identification to the the surgery.

• Please note processing registrations can take up to 1 month.
• By completing this form, you are automatically opted-in for us to contact you on any communication method you share with us, including Phone calls, Voicemails, SMS, and Emails. If you wish to opt-out of any of these methods, please advise reception.
